Applications for 2024-2025 School Year will be available online on October 24, 2023 at 9:00 a.m.

The following are Required Forms when applying online to Our Lady of Fatima Catholic School:

Birth Certificate - REQUIRED FOR ALL
Baptismal Certificate - REQUIRED FOR ALL
Immunization Record - REQUIRED FOR ALL
**The immunization record must be recorded by a Physician on a Louisiana State Certification of Immunization Card or Printout from your child's pediatrician.**
Current Progress Report and/or Progress Reports from all prior school year(s) - Only grades 1-8
Standardized Test Scores from each year the test was taken - Only grades 2-8

**Required for all except Our Lady of Fatima Parishioners.**
Online Applications do have an assessed fee of $25 for the application to be submitted completely.
This fee is non-refundable.
