The school lunch program is mandatory for all students as directed by Diocesan policy unless a student has a dietary requirement that the cafeteria is unable to meet and the following prescription is completed by a physician (not related to the student).  This form must be filled out and returned with the doctor's excuse to your child's homeroom teacher at the beginning of each school year.

If your child has been diagnosed by their doctor with a food allergy that necessitates bringing his/her own lunch; please submit both the prescription form above and a doctor's excuse from their doctor to his/her teacher and the cafeteria manager.  Once we have the proper documentation on file they must bring their lunch consistently everyday after; so that the cafeteria has a more accurate number of lunches to prepare each day.
